UK The Hazardous Waste Regulations 2005

The waste collection process produces three identical signed documents for each of the following recipients:

  1. Producer/Holder/Consignor
  2. Carrier
  3. Consignee

Part A: Notification Details

  • Consignment note code
  • The waste described below is to be removed from (name, address, postcode, telephone, e-mail, facsimile)
  • The waste will be taken to (name, address and postcode)
  • The waste producer was (if different from 2) (name, address, postcode, telephone, e-mail, facsimile)

Part B: Description of the waste

  • The process giving rise to the waste(s) was
  • SIC (2007) for the process giving rise to the waste
  • Waste Details: More than one waste type? Complete for each EWC identified.

Part C: Carrier’s certificate

  • Carriername (and signature): On behalf of (name, address, postcode, telephone, e-mail, facsimile)
  • Carrier registration no./ reason for exemption
  • Vehicle registration no. (or mode of transport, if not road)

Part D: Consignor’s certificate

  • I certify that the information ....
  • I confirm that I have fulfilled my duty ....

Part E: Consignee's certificate

  • I received this waste at the address given in A3 on [date] and [time]
